Bill 25 is a Quebec law designed to strengthen the protection of privacy and personal information. The protection of this data is paramount, and we comply with the law.

In the course of our activities, information collected and used from patients, employees and suppliers is subject to the application of this law, and is protected.

Our policy is designed to govern the collection of your personal information, i.e., how it is communicated, stored and destroyed.

In accordance with this law, our policy also aims to inform all interested parties about the practices of Clinique médicale de l’Alternative with regard to the protection of your personal data. Our policy applies to personal information, regardless of how the information is collected, whether automatically in person, by telephone, by text message, by fax or electronically.

In general, the Clinique médicale de l’Alternative obtains patients’ consent to data collection by explaining the purpose of the collection.

In this case, when patients or employees are on site, they are given a data collection form and a consent form. For telephone appointments, consent is given verbally.

Our policy is to ensure that all employees are trained and made aware of the need to protect personal information.

The person assigned to secretarial duties will have your identifying information such as your name, address, telephone number, date of birth and health insurance number, but will not have access to your medical file. Only medical personnel, such as nurses, care providers or physicians, have access to your personal information concerning your medical file.

We collect your personal information either by form if the patient is in person, or by verbal consent if by telephone. We use fax to send results securely only if it is a secure medical facility or secure organization. We also use the DSQ (Quebec Health Record).

We inform you of the purposes for which your personal information is collected, of your right to access or rectify your personal data, as provided by Bill 25, and of your right to withdraw your consent. Everyone has the right to consent or not to the collection of personal information.

For patients of Clinique médicale de l’Alternative, information collected using Purkinje computer software is stored on an internal and external server, as well as in a paper file that is filed and archived for a period of 5 years.

We restrict access to the archives and use of the DSQ device to medical and administrative staff only. Use of the DSQ is restricted to nurses, doctors or health care professionals.

Each doctor, nurse or health care provider has their own device, USB key with their own password, different for each provider. The use of the device with a unique password must be approved by the Ministère de la santé du Québec.

Personal information is information about an individual that directly or indirectly identifies you.

The reason we collect your personal information is to verify your identity in order to provide you with the best possible service. The use of your personal information is in accordance with ethical laws and the recommendations of the Collège des médecins du Québec and the Ordre des infirmiers et infirmières du Québec.

Personal data collected on employees and suppliers is used for administrative purposes only. Only administrative personnel have access to this personal information.

If you do not consent to identification, it will not be possible to offer you services.

Your personal identification information is shared with RAMQ billing personnel or Ciusss de Montréal personnel.

The billing and identification software is supported by Purkinje’s Facturation.net. Our external server containing your personal data is managed by GTO informatique, who are also subject to Bill 25 on the confidentiality of personal information.

All of Clinique médicale de l’Alternative’s security standards are in place to protect your personal information, whether at the medical, administrative or computer support level, in order to avoid any confidentiality incident.

In spite of all the security measures put in place to protect your personal information, we must inform you that there is a risk in computer use, and that the use of your personal information is at your own risk.

Rest assured that we are committed to providing a secure environment for your personal information to minimize the risk of data leakage.

Our staff will only use your personal information for medical or administrative purposes, except in the case of an emergency that threatens your safety or the safety of others. In this case, we would notify the police.

Other situations may require the transmission of certain personal information, such as in the case of fraud, to provide a service, at your request or in the context of criminal or penal proceedings.

Please be aware that you have the right to access your personal information, and that your information may be communicated to you or transferred. Your rights also apply in the event that you wish to rectify your personal data because it is inaccurate or has changed.

You have the right to withdraw your consent for your personal data at any time. You may also restrict access to certain parties so that they no longer have access to your information.

If you believe that a violation has occurred, you have the right to file a complaint with the person responsible for the protection of personal information or with the Commission d’accès à l’information du Québec.
